Assisted Living - Patients and Families

The more information we have the more effectively we can manage your health.
Medicare Advantage plans require the PCP to be assigned as Dr George Valdez before we are able to assume care and establish a relationship or prescribe any medications.  The sooner this is accomplished, the sooner we can assume care.
Healthcare is built disconnected. It takes active, ongoing attention and effort to optimize outcomes.
Please notify us through the Care Hub with any visits to hospital, ER, specialist, or any changes you in status you are aware of.

Home health:  Assisted Living is just that. Seniors live on their own, with extra eyes and ears and hands to help them out, but they often do not provide skilled medical services. Home health as often needed to help organize medications check lab work take care of wounds and other skilled nursing services. They may also provide physical or occupational therapy as needed to improve balance, strength, and safety. You make the decision on which company takes care of your love one. We encourage you to review quality ratings through the Medicare.GOV website. There are a multitude of companies, and a great variety and quality. Dr. Valdez is associate medical director for Carter Home Health and Complete Nurse Solutions home health, and receives a flat fee for services.  There is no incentive to refer patients, but by acting as medical director, he has clearer communication with the care team members, as well as closer oversight of quality of care.

Hospice:  Hospice is a type of medical care which becomes appropriate towards the end of life. Hospice teams include physician, nurses, social workers, religious figures, and a team director. They meet regularly all together for each patient to make sure quality of life is optimized, even for patients who cannot speak for themselves. The best time to discuss and find out what Hospice is and isn’t, is prior to a major decline in status or function. It is a difficult part of life, but when it is done correctly by a good team, can make an incredible difference for patients and family members. Again, the patient and family are in charge of which company to use. Dr. Valdez is Board certified in Hospice and Palliative care.   He is associate medical director for Rose of Texas Hospice.  Hospice care requires a medical director to be involved in all decisions. If another company is involved, they may have an excellent director, but it is similar to having to great chefs for one recipe, and often doesn’t work well. If a patient or family chooses another company, Dr. Valdez will turn over all care to that company’s medical director to avoid any confusion in care.

Medications:  We do our best to minimize and optimize meds. Please review our list through the Patient Portal and compare to the Facility list to make sure they match. Again it takes all of us and ongoing effort to ensure accuracy. If you feel a medication is unnecessary, please notify us.  If you feel a medication is unaffordable, we do care and will do our best to find an affordable solution. We may know of a cheap generic offhand, but we do not do pre-authorizations, and we ask you make the effort to research or ask the patient’s part D plan, which brand is preferred by them.  We used to be able to take care of this, but there are a multitude of plans so it is no longer feasible to keep track of.
